For many teachers under Kenya’s Teachers Service Commission (TSC), the process of filing a medical claim can be daunting.
TSC provides a clear process for teachers and their dependents to claim medical reimbursements.
You can file for yourself, your spouse, or your children. Understanding the requirements and procedure is essential to ensure a smooth approval.
Step 1: Obtain the Official Medical Claim Form
Teachers must first obtain the TSC Medical Claim Form (Revised), referenced as TSC/Med/3.
The form is available from the Teachers Service Commission offices or the TSC website.
It must be completed in duplicate and submitted alongside all required documents.
Step 2: Fill Out Your Personal Details
In Part I, teachers must provide their full name, school, address, monthly salary, and TSC number.

The section also includes a declaration of who the claim covers, whether it’s the teacher, spouse (as listed on the NHIF card), or children.
Teachers should list the names and ages of dependents for whom the expenses were incurred.
Step 3: Choose Your Payment Option
Under Part II: Pay-Point, teachers select how they wish to receive reimbursement.
Payment can be made through a specified bank account (including the bank name, branch, and account number) or through the District Education Officer (DEO).
Step 4: Special Section for Married Women
Part III applies only to married female teachers. They must confirm that their husbands are not teachers or public servants.
If the husband is employed, his employer’s name should be provided.
This declaration ensures there is no double claiming from other public medical benefit programs.
Step 5: Certify and Attach Proof
In Part IV, the claimant certifies that the amount was incurred personally and attaches receipts, invoices, and hospital statements.

The teacher must indicate the hospital’s name and sign the declaration.
The school principal or headteacher must then complete Part V, confirming that the claimant is actively teaching in the institution and recommending the payment.
Finally, the PDE/DEO certifies the teacher’s employment status and endorses the claim in Part VI.
Step 6: Ensure All Attachments Are Correct
The notes section outlines crucial documentation rules:
- All cash receipts must be original, stamped, and affixed with the hospital seal.
- Invoices or statements must show dates of admission and discharge, as well as the NHIF reference number.
- Cash sale receipts for prescribed drugs must carry the hospital’s rubber stamp.
- Supporting documents are required for dependents, a marriage certificate or affidavit for a spouse, and birth certificates or notifications for children.
- A sick sheet or sick leave form must be attached if the teacher was admitted for more than 14 days.
- A copy of the latest pay slip is mandatory for every claim.
Step 7: Submit and Follow Up
Once the form is complete and signed by the relevant authorities, teachers should submit it through their school head or the District Education Office.
Keeping a copy of the entire claim package helps in tracking the process.
